Thank you for participating in the campaign, Thriving with CRI! Your stories and photos help us celebrate the incredible work you do and the impact our consumers experience every day

Follow these simple steps to submit your content:

    1. Fill Out the My Care Experience activity sheet – Use the worksheet to capture a fun story with your consumer.
    2. Snap pictures of your consumer with the #ThrivingWithCRI photo prop.
    3. Check the Model Release – Ensure the back of the activity sheet is signed.
    4. Upload Your Files: photo of My Care Experience activity, model release, and selfies.
    5. Share Anything Extra (Optional) – Add a quick note about your experience!
    6. Click ‘Submit’: That’s it!
